Testicular Placental Site Trophoblastic Tumour

概述：

为非绒毛膜癌性滋养层细胞肿瘤的一个亚型。

发病部位： 睾丸

诊断要点：

1. 罕见，目前仅有少数病例报道，通常作为混合性生殖细胞肿瘤中很小的成分，无特定的大体形态学特征；
2. 表现为单个或疏松、无黏附性的植入性中间型滋养层细胞浸润性生长，细胞胞质丰富，嗜酸性或局部空泡变，核不规则，粉尘状深染，典型病例浸润血管平滑肌伴纤维素变性。

免疫组织化学染色：

表达滋养层细胞标记物，如 3-β-hydroxysteroid dehydrogenase(β-羟类固醇脱氢酶)、inhibin、GATA3. CK18，人类胎盘催乳素（HPL）阳性，p63 阴性。

分子标记：

i12p

鉴别诊断：

1. 上皮样滋养细胞肿瘤：瘤细胞之间可将丰富的透明变基质，免疫组化染色弥漫表达 P63。

2. 胚胎性癌：瘤细胞呈合体状生长，免疫组化染色表达 OCT3/4，不表达 GATA3。

3. 绒毛膜癌：有细胞滋养层样和合体滋养层样细胞组成，常见出血、坏死。

治疗：

睾丸切除
